12/1/2023 EnchantaVaughn Approved QMHPA greeted Ms. Cosby upon arrival to the home setting. Upon arrival to the home setting, Ms. Cosby appeared to be content mood evident by her facial expressions such as smile on the face. QMHPA inquired about her afternoon. Ms. Cosby verbalized to QMHPA that her afternoon was going well. QMHPA informed Ms. Cosby that this session was going to continue to focus on reducing depressive and anxiety symptoms. Ms. Cosby did not present any symptoms of SI or HI at the time of the assessment. QMHPA began the session by inquiring about Ms. Cosbyâs current thoughts, feelings, and emotions. QMHPA engaged Ms. Cosby in discussion surrounding her medication management. QMHPA engaged Ms. Cosby in a skills building activity. QMHPA explained to Ms. Cosby that the purpose of this activity was to teach skills to overcoming depression, anxiety and helping her to feel good about herself. QMHPA processed with Ms. Cosby on her readings. QMHPA engaged Ms. Cosby in another skills building activity. QMHPA explained to Ms. Cosby that the purpose of this activity was to assist her with taking responsibility for her actions and recognizing acceptable and unacceptable behaviors. QMHPA engaged Ms. Cosby in reality situations when it comes to her personal relationship, social life, and relationship with her daughter. QMHPA processed with Ms. Cosby on journaling her feelings and thoughts to support with reducing being angry and overwhelmed about people, situations and problems. Ms. Cosby appeared to be receptive to QMHPAsâ interventions and services that were provided to her during the session. Ms. Cosby reported that she sometimes feels hopeless and scared about her life and the things she's going through and continues to go through daily. Ms. Cosby indicated to QMHPA that she takes her medication when she wants to take it but she knows that it is not right. Ms. Cosby verbalized to QMHPA that she really needs the medication because she can tell the difference when she is on her medication verses when she is not on her medication. During the skills building activity, Ms. Cosby was able to provide QMHPA with ways that she can overcome depression. She indicated to QMHPA that she doesnât need to involve herself into others business that does not contain her because she notices a lot of stress and depression that gets her down and to the point where she feels as though she needs to go to a hospital and that is a feeling that she doesnât like.
